Integrated Practice - improving the way we help and support children and young people

The way organisations work together to help children and young people in Hertfordshire is changing.
The changes will mean that everyone who works with children and young people, including teachers, doctors and social workers, will need to work together more closely. This will mean that children and young people get the extra help and support they need, but don't have to keep repeating their story to lots of different people.
These changes are all part of the Government's
Every Child Matters: Change for Children programme to improve the services we deliver to children, young people and their families.
The tools include ContactPoint, the Common Assessment Framework (CAF)and the Lead Professional role. This section will tell you what these tools are and how they will be used by those who work with children across the county.
