Transition Plan The plan made at the first annual review after a pupils 14th birthday, to begin to establish the future plans, as a pupil with SEN approaches the time to leave school. It sets out the young persons needs in the future as they move from school to further education or training and then to adult life in society. The transition plan is then reviewed with the statement each year.
