When you are in hospital……. A social care service for Hemel Hempstead General Hospital and St Albans City Hospital

The service is for

Patients of all ages staying in hospital or attending day hospital or out-patient clinics, their families and carers.

Parents, children and families at ante-natal clinics, maternity and children’s wards.

Those attending the Accident and Emergency department.

People who will be returning home after a stay in hospital.

People who will be moving to a residential or nursing care home.

What we can offer

Information – on a wide range of social care services.

Help – to plan for coming into and leaving hospital and for ongoing care and support.

Advice – on welfare benefits, housing and other problems.

You can contact us yourself or ask a relative, friend or carer to contact us on your behalf.
We have offices at each hospital with two specialist teams, one working with children and their families and the other with adults.

We are available between 9.00 am and 5.20 pm, Monday to Thursday, 9.00 am and 4.30 pm on Friday.
