I am from an organisation and would like to apply for a badge (or renew one)

To make an application or a renewal you can:

• contact us and ask for an application form to be sent to you. Select the link called 'Contact us' under 'Site tools' on the left hand side and use the Adult Care Services number.
• print the application form attached to this page.

Send the completed form together with a copy of your organisation's logo and a payment of £2.00 for each badge required - cheque or postal order payable to Hertfordshire County Council - to the address given on the form.
