I would like to apply for a badge (or renew one)
To make an application or a renewal you can:
- contact us and ask for an application form to be sent to you. Select the link called 'Contact us' under 'Site tools' on the left hand side of the page, and use the Adult Care Services number.
- print the application form attached to this page.
Send the completed form, together with a passport-size photograph of yourself with your name printed on the back, a cheque or postal order for £2 - payable to Hertfordshire County Council (to cover administration costs) and any supporting paperwork to the address given on the form.
If we cannot issue a badge we will return the fee and the photograph.
Please do not send Post Office books for payment of allowances in the post as proof that you receive the higher rate of the mobility component of Disability Living Allowance and the War Pensioners' Mobility Supplement. Photocopies are acceptable or an official letter stating your allowance, your Vehicle Excise Duty (Car Tax) exemption certificate or evidence that you have obtained a vehicle through Motability.
Please do not send the original - just a photocopy.
Please note this form is for individuals applying for the blue badge, not institutions. The form should be completed for the person requiring the badge and not the driver of the vehicle.
