You can ask for help for yourself or someone else. If you are contacting us to ask for help for someone else, it is important that they agree to you acting on their behalf. We will ask you about this.

If you are contacting us on behalf of someone else these questions refer to them. We also want to hear your own views of their situation particularly where you are involved in looking after them.

Your online form will be looked at during office hours - 8am to 8pm weekdays and 9am to 4pm Saturdays. Our client services staff will contact you to let you know what will happen next and to ask for more details if necessary.
