Having your say form

The Having Your Say form asks some simple questions about how satisfied users and/or carers are with us and how well we have listened to your needs and worked with you.

Adult Care Services staff should give you the form at your assessment and/or your review or post it on to you afterwards. If you attend any regular services, such as a day centre, you should be able to pick one up as and when you wish. If you are not offered the form please ask for it, or let us know on the number below, and we will make sure you get one.

You do not have to put your contact details onto the form, but if you want us to follow something up it may help if we can discuss it with you first. The form is post-paid and is returned to the Users and Carers team in the Performance and Workforce Development Unit.

In 2006-07, the team received around 2,500 forms, which showed generally high levels of satisfaction, and provided many compliments about members of staff. We were also able to support teams and services in following up individual difficulties. The process has helped us improve customer care across the department.

If you cannot fill in the form yourself, you can ask a friend or similar independent person to help you. If you have an interpreter for the assessment or review, you can ask them to stay and help you complete the form.

You can also contact the Users and Carers team on 01707 280752 (a dedicated line and answer phone) and we will ring you back and fill it in over the phone at our expense. If you prefer someone independent of the department to talk to we can suggest a relevant voluntary organisation that might be able to help you.

There is no pressure to fill the form in. Not filling it in will not affect the service you receive.
