Support from Health and Community Services
We provide advice, support and care to older people, people with learning disabilities, physical disabilities or sensory needs and other adults who are vulnerable and would have difficulty living independently without some support.
We support people with mental health problems through Hertfordshire Partnership NHS Foundation Trust which operates integrated health and social care services.
We can arrange and fund support for adults when there is a critical or substantial risk to their independence worked out by assessing needs against nationally determined eligibility criteria.
We offer to assess carers' needs to see if we can support them in their caring role and to ensure their own needs are met.
Once we have assessed someone's needs, we will work with them and their families to agree how to meet these needs. This can be by receiving a sum of money to arrange their own care and support, having all their care arranged by social workers and other staff or a mix of a payment and organised care.
We can also advise people about their support and care needs.
