Our Hospital Discharge & Independent Living Scheme aims to support an older person who has recently been discharged from hospital to help them to readjust to living independently again. A volunteer visits the older person at home and can:
- Provide social support and information on local services
- Do basic shopping and prepare sandwiches and drinks
- Collect prescriptions and take people to collect pensions
- Help with gentle mobilisation, such as walking short distances
- Will visit on a regular basis, usually once a week
By providing the patient with both a friend and health information, it is hoped that readmittance to hospital will be delayed or prevented.
Criteria for the scheme:
- Pateints aged 60 or over
- Those living alone during the day, or living with an elderly or frail partner
- Patients who are frail, have poor mobility or who are housebound
- Patients who have sensory impairment
- Patients in need of social support
