Age Concern Hertfordshire. Hospital Discharge & Independent Living Scheme

Our Hospital Discharge & Independent Living Scheme aims to support an older person who has recently been discharged from hospital to help them to readjust to living independently again. A volunteer visits the older person at home and can:

• Provide social support and information on local services
• Do basic shopping and prepare sandwiches and drinks
• Collect prescriptions and take people to collect pensions
• Help with gentle mobilisation, such as walking short distances
• Will visit on a regular basis, usually once a week

• Pateints aged 60 or over
• Those living alone during the day, or living with an elderly or frail partner
• Patients who are frail, have poor mobility or who are housebound
• Patients who have sensory impairment
• Patients in need of social support
