Support link offers intensive community support and assertive outreach support to people in West Hertfordshire who are experiencing serve and enduring mental health difficulties and have a history of associated offending behaviour and/or difficulties with drug/alcohol misuse.
We provide support for people who have complex needs and may find it difficult to engage with mainstream services. This may be due to their need for an outreach based service, or because they have had difficult experiences with services in the past.
Our project workers maintain regular contact by visiting service users in their homes and carefully selected community settings. We aim to support service users to develop the necessary skills and understanding to improve their quality of life within the community. We achieve this by tailoring our support to meet individual needs.
Support Link draws up a personal support plan with the service user to provide a range of support and advice in the following areas:
* Building social and life skills
* Budgeting and Benefits
* Support accessing other services
* Housing
* Health
* Emotional support
* Employment or voluntary work
* Education and training
We draw up a support plan with all our service users to identify what types of support will be beneficial. Support plans are reviewed every three months to ensure that we continue to respond to changing needs.
