The Hertfordshire Children's and Young People Palliative Care Service aims to provide a service for children and young people with life limiting conditions and their families, across Hertfordshire.

Services available include:

* Liason with Children's Community Nurses, GP's and specialists.
* Support for siblings.
* Advice, support and information for families and professionals.

Anyone can make a referral to this service. Please contact for further information.
