The Access - Ability Team is part of Connexions Hertfordshire and works with young people with learning difficulties and or other disabilities from the age of 13 up to 25. Young people are supported by Personal Advisers who can help young people make successful transistions from school to further education and training. We also support young people looking for employment.
Personal advisers start working with young people while they are at school. They may see them in groups or individually. The Advisers can help young people think about what they want from their future and help address any barriers which may prevent them fullfilling their ambitions in the future. Personal Advisers will attend annual reviews for young people with Statements of Special Educational Needs at school, and will contribute to a young person's Transition Plan by completing a report for transition.
Personal Advisers will continue to support young people once they have left school. This can be to 25.
