Transition to adulthood and leaving school - guidance for parents and carers of children with Special Educational Needs (SEN)

Connexions

Connexions Hertfordshire offers support to all young people to enable them to make successful transitions to adulthood. For young people with Learning Difficulties and Disabilities this support can be accessed up to the age of 25 years where needed. The support includes:
  • Assessment of a young persons needs
  • Impartial guidance and support from a Personal Advisor
  • Careers Guidance
  • Help to consider work, training and college options
There is a dedicated team of Personal Advisors called Access Ability within Connexions who can help any young person in a special school and who also support other Personal Advisors working with young disabled people.

If your child has a Statement of Special Educational Needs, a Connexions Personal Advisor will start to work with your child from Year 9. The Personal Advisor will draw up a plan for what your child will do when they leave school and help you and your child to implement this plan. Personal Advisors will also attend certain annual reviews.
