How to make a referral

Important referral Information

(Add info about how to make a referral if you are a member of the public or a professional.)

Before making a referral it is important to note the following:

Referral Letters should contain:
    Basic data re referred child;
  • NHS number
  • Date of birth
  • Address
  • Telephone number
  • Name of GP
    Description of problem and family including;
  • A description of the problem, how long it has lasted and where it occurs
  • Has this child or family attended CAMHS before?
  • The family composition and any recent life events
  • A description of the community interventions already tried or a statement why this would be inappropriate
  • A list of all professionals involved with names and contact numbers

In addition a Referrer PRIOR to referral should have:
  • Seen the child and discussed the referral with them (appropriate to their age)
  • Seen the family, offered advice such as community sources of support / help that might be accesses and met with the family to review the outcome of that advice

Please note: referrals where the child has not been seen will not be accepted.
